Description of Role:

We are looking to hire a motivated and enthusiastic individual to fill the position of Community Health Worker/Resource Coordinator

The Community Health Worker/Resource Coordinator is responsible for coordinated referral systems and the support of school based PBIS. The Community Health Worker/Resource Coordinator will work to directly impact and increase academic outcomes by addressing barriers to resources that exist within the child, school, home, or community.

Compensation:

• Pay: Compensation is $17.00 per hour. Position is Grant funded and is not guaranteed

• Full time (100% FTE): 160 hours annual PTO

Knowledge and Abilities:

The position requires a Community Health Worker Certification and or High School Diploma

Principle Responsibilities:

Work closely with school counselor and staff to ensure coordination and wrap around services for students and families

• Identify available community resources

• Provide referrals for students and families

• Family engagement through referrals, school, and community events

• Participation in partner meetings

• Participation in school based PBIS teams and MDT
