Lyon County communities – Healthy Communities Coalition is beginning the process of writing our next 3-year Comprehensive Community Prevention Plan (CCPP) and we need your help.

Healthy Communities has created a short Community Wellness Survey and we value your input. Your responses will provide us with a way to gauge how the community views prevention and other services in the area and how we should move forward over the next few years.

The purpose of the survey is to understand the overall community wellness and access to health care in Lyon County, Nevada. Your answers will be used by Healthy Communities Coalition staff to help determine the impact and the needs of the community. Your answers will be kept confidential. This survey should take 5 minutes or less of your time.
